Life experiences and design history for documentation. My brain does the rest. It's kind of like a blender...
Everything. Anything I come in contact or interact with usually gets filed away in my head and I draw upon that when I start a project. If it's something new, I will do further research.
When I'm surfing in my free time, I do tend to bookmark sites I think are well designed. I also have a few design shop sites bookmarked and look at their work from time to time to get inspired.

Collect all the information required from the customer, create a question and answer paper tha cointains the information collected form the custom preliminary meeting, send it back to the customer and make sure that me and my team collected all the information correctly and fully. Then I work on paper and pencil. then brain storming

Ideas are generated by research into the clients work and their customers. I rarely follow current trends. If necessary I will look at a clients industry to see how rival businesses advertise themselves, and also look into the demographic of a typical customer.

I start with a blank pad, then once I have exhausted these I go to books, old work, anywhere that I can pick out I nice detail, careful to only be inspired by the detail and follow the mind set, not to take direct work.
Depends on the job. I like to hit the library or the bookstore whenever a new job starts. Researching the subject-matter always proves fruitful for ideas and inspiration.
The sources are everywhere, I try to keep my eyes and ears open, be present in my time and updated on current events, not so much in my own field of design but in other creative areas, looking into both old and new work. Traveling is giving me the best ideas and source of inspiration for my own work. Music and movies fill my life with a lot of inspiration.
