Not eating at all.
The main error is that you kid yourself that you are still on a diet! Most slimmers cut calorie intake too much then give up after a couple of weeks.
It's better to reduce calories to 1500-1800 per day and stick to it for at least 6 months. Boost physical activity levels to 60 minutes per day (any type of exercise will do). Then you should lose 0.5-1kg (1-2lbs) per week.
Starting them in the first place! Diets generally don't have a long term effect, better I think to try to understand your relationship with food and what emotional triggers are affecting how you eat.
People cheat. Most diets fail because people could not put up with them forever. A diet is not just for a few weeks or to lose a dress size for a wedding... You should eat every day in the most healthy way to maximize your 'health span' rather than your life span.
You should also have the occasional Zag day....

It is right there in the question. They are diets. The minute we say we're going on a diet we are setting ourselves up for failure. We should be striving for a lifestyle change. Once you make it about changing your lifestyle then you are making a commit to yourself forever.
If it came in a pill or capsule or even if it was simple, we would all have perfect bodes.
