How should a work of art be evaluated?
Not a big fan of "should." If someone were viewing my work, I'd hope that it would inspire the viewer in some way.
By its creativity, how crazy can you get?
If it pleases the viewer, it's good. If the artist says it's art, it is.
That's a complex question.
By the way it "communicates" with the viewer.
Art shouldn't be evaluated. Art is a way of life.
To me if I am happy with my work then it's fine. Sometimes I see things in other peoples work that make my mistakes glaringingly obvious when I come back to the studio, and I spend some time re-evaluating them, but my aim is to be the best i can be and I have to take that statement on a daily basis.

it shouldn't be evaluated just acknowledged... wait scratch that, instead art should be evaluated by motive, why you created, not what.
Effectively, only with the passage of time
You should evaluate art based on how it makes you feel. If you don't feel anything when interacting with it, good or bad, move on until you do.
Does it amuse, entertain, instigate, move or facilitate?
When it 'speaks' to you in a personal way
Evaluated- sorry I cannot judge this word.
